Vitrectomy Procedure: Step-by-Step Explained

1. Pre-Vitrectomy Surgery Preparation

Before the procedure begins:

  • Your eye is thoroughly cleaned to prevent infection.

  • Local anesthesia (numbing injection) or sometimes general anesthesia is given so you don’t feel pain.

  • A device may be used to keep your eye open during surgery.


2. Creating Tiny Incisions

  • The surgeon makes very small cuts (about 0.5 mm) in the white part of your eye (sclera).

  • These incisions allow entry for microscopic surgical instruments.


3. Inserting Surgical Instruments

Three main tools are inserted:

  • A light source to illuminate the inside of the eye

  • A vitrector (cutting and suction tool)

  • An infusion line to maintain eye pressure with fluid


4. Removal of the Vitreous Gel

  • The vitreous (gel inside the eye) is carefully removed using the vitrector.

  • This step gives the surgeon a clear view and access to the retina.


5. Treating the Underlying Eye Problem

Depending on your condition, the surgeon may:

  • Repair a retinal detachment

  • Remove scar tissue or membranes

  • Treat bleeding (vitreous hemorrhage)

  • Use a laser to seal retinal tears

  • Close a macular hole


6. Replacing the Vitreous

After treatment, the vitreous is replaced with one of the following:

  • Saline solution (most common)

  • Gas bubble (helps hold the retina in place)

  • Silicone oil (used in more complex cases)


7. Closing the Incisions

  • The tiny incisions are usually self-sealing and may not need stitches.

  • If necessary, very fine sutures are used.


8. Post-Surgery Care

  • An eye patch or shield is placed for protection.

  • You’ll be monitored briefly before going home the same day (in most cases).


Important Aftercare Instructions

  • Use prescribed eye drops regularly

  • Avoid rubbing or pressing the eye

  • Follow special head positioning if a gas bubble is used

  • Avoid air travel until the gas bubble dissolves

  • Attend all follow-up visits


How Long Does It Take?

  • The surgery typically lasts 1 to 2 hours, depending on complexity.


What You Might Feel After Surgery

  • Mild discomfort or irritation

  • Blurry vision initially

  • Gradual improvement over weeks
